Credentialed Mental Health Nurse Service
The Credentialed Mental Health Nurse Service provides ongoing therapeutic interventions and coordination of clinical services for people with enduring and complex mental illness which impacts on their social, personal and work life.
A Credentialed Mental Health Nurse can provide regular reviews, medication monitoring, physical healthcare information and liaison with carers and relevant support service as required.
Credentialed Mental Health Nurses provide services across South Western Sydney.

Operating Hours
Central intake is open Monday - Friday 8.30am - 4.30pm. Hours of service delivery are dependent on the clinicians availability and your needs

Ways to make a referral
See your GP
Referral Process
Your GP will complete a Referral and Mental Health Treatment Plan (should you not already have one in place) and fax this to Central Intake.
Your referral will be processed and allocated to a mental health nurse who will contact you to schedule your first appointment.
